Tell us what the concern relates to
Identify the product, award reference, entry or public statement. For an appeal against your own decision, use the procedural appeal route in your account.
Provide the relevant facts
Explain what happened, where it can be checked and why it concerns you. Include public links or document references. Do not send unnecessary personal information, allegations about unrelated parties or confidential patient data.
How we examine a report
The awards team checks the report against the relevant application, decision or permission. An independence concern is referred to a person who is not involved in the matter. Further information may be requested. Reporting a concern does not itself establish wrongdoing.
What you can expect
Keep the reference shown after submission. Where a record or permission requires a change, the register is updated accordingly. We may be unable to disclose confidential evidence, personnel information or details of an ongoing assessment.
